Housebound Annual Review System 

You are housebound if you are unable to leave your home. This means that your annual review tests will take place at home, usually by the community nursing team.

The practice pharmacist will then contact you to review your medication with you, and reauthorise it as appropriate.

Your Practice Nurse may then contact you to review your current health issues and plan for the next year.

Home visits

The doctors can carry out home visits for patients who are housebound and who are more vulnerable.

We ask you to call before 11.30 am to arrange this so that we can assess the need for a visit the same day. However, this is at the doctor’s discretion and they will assess your situation over the telephone and advise you appropriately.
