How To Register

To register with the practice you will need to fill out an application form and it would be helpful if you could provide proof of identity such as your passport/ID card and your NHS Number. You may then be offered a New Patient appointment with our Health Care Assistant. It also provides an important opportunity to ensure that we are fully aware of your present medical requirements, as it may be some time before we receive your medical records.

Please collect a registration form from the practice. This is also available to download https://gp-registration.nhs.uk/register-with-a-gp-surgery-paper-formPrint and fill in the form and return it to the practice to register as it can’t be completed online.


Click this link https://gp-registration.nhs.uk/C88027/gpregistration/landing for the new way patients can register for our surgery.  It automatically checks your address to see if it is in our catchment area and tells you if its not.  You can ask to register but if you are outside our Practice area we will have to reject you as we do not operate an 'Out of Area' registration. We cover areas (but not all of), S4, S5 and S9 postcodes.
 

Overseas Visitors

Please note that overseas visitors are entitled to Free NHS GP services once they register with the practice. Hospital services apart from A&E may attract a charge.
